Velká Británie definitivně odchází z EU. Jaké změny nás v souvislosti s Brexitem od ledna čekají?
Od ledna 2021 přestává být Velká Británie členem Evropské unie (EU). Končí přechodné období. Brexit nabývá konkrétní podoby, i když řada věcí zůstává zatím nevyjasněná. Co víme už nyní a jaké změny v oblasti zdravotního pojištění Brexit přinese?

- Britským turistům končí platnost evropských průkazů zdravotního pojištění
Britským turistům a osobám pojištěným ve Spojeném království (UK) skončí po 1. lednu 2021 platnost stávajících Evropských průkazů zdravotního pojištění, takže je již nebude možné na základě tohoto průkazu ošetřit.
Průkazy Evropské unie nahradí nové s hologramem britské vlaječky a označením CRA. Vystavování těchto průkazů začalo v listopadu 2020, přičemž se dá o průkaz zažádat i online na odkazu NHSBSA zde. Online cestou mohou pojištěnci zažádat i o náhradní, papírové potvrzení nároku na zdravotní péči.
- S jakým průkazem mají cestovat do Británie Češi?
Odpověď na tuto otázku patří mezi ty, které jsou dosud nevyjasněné. Aktuální informace budeme průběžně zveřejňovat na webu ZP MV ČR, zároveň radíme sledovat internetové stránky Kanceláře ZP zde.
Pokud se chystáte vycestovat do UK na přelomu roku nebo během ledna, doporučujeme vám obrátit se na ZP MV ČR pro zjištění aktuálních informací o možnosti využití Evropského průkazu zdravotního pojištění nebo zajištění speciálního potvrzení o nároku určeného pouze pro Spojené království.
Zároveň vám doporučujeme, abyste před cestou do Velké Británie uzavřeli cestovní pojištění, jako byste cestovali mimo země EU. To pro případ, že byste museli vynaložit náklady na zdravotní péči sami a ZP MV ČR by vám je následně nemohla refundovat.
- Jste britský občan a pracujete či podnikáte v České republice?
Podle výstupové dohody pro Brexit je klíčové, jestli vaše zaměstnání trvalo i před 1. lednem 2021. Pro britské občany, u kterých zaměstnání v ČR trvá od roku 2020 či déle, se nic nemění, a to ani po dobu trvání zaměstnaneckého poměru, ani po jeho ukončení. Z výstupové dohody plyne, že i v případě, kdy zaměstnání ukončíte, i nadále na vás platí principy vyplývající z členství v EU, například můžete být v ČR pojištěni na základě bydliště, bez nutnosti povolení k trvalému pobytu.
Začnete-li v České republice pracovat až v roce 2021, výstupová dohoda se na tuto situaci nevztahuje a po ukončení zaměstnání můžete setrvat v českém systému zdravotního pojištění pouze na základě trvalého pobytu.
Analogická situace platí pro britské OSVČ – pokud jste v ČR podnikali už v roce 2020 či dříve, nic se pro vás nemění, a to ani za předpokladu ukončení podnikání.
Pakliže začnete v České republice podnikat až po 1. lednu 2021, výstupová dohoda se na vás nevztahuje, může na vás být tedy nahlíženo jako na cizince ze zemí mimo EU, jestliže nedojde k úpravě těchto práv například uzavřením zvláštní smlouvy mezi EU a UK.
- Informace pro britské pojištěnce, kteří jsou registrováni u ZP MV ČR
Jestliže jste v ČR registrováni u ZP MV ČR na základě formuláře S1 nebo případně E106 či E109, který vám byl vystaven institucí ve Spojeném království, situace se pro vás nemění a uvedená registrace v ČR a nároky na plnou nebo plánovanou zdravotní péči v ČR jsou zachovány.
Pokud vám byl v UK vystaven nárokový doklad S2 za účelem plánované péče v ČR a tento doklad byl vystaven před 1. 1. 2021, zůstává tento formulář v platnosti po celou dobu, na kterou byl vystaven.
What changes await us after 1 January in connection with Brexit?
As 2020 draws to a close, so does the transition period for Great Britain’s exit from the European Union – this will bring changes, some of which affect also the area of individual health insurance. What does this mean for you, as a policyholder who is insured with the Health Insurance Company of the Czech Ministry of the Interior (ZP MV ČR)? We have prepared a straightforward overview of the latest development for you, in which we are also addressing the question of what matters remain uncertain for now and thus require your heightened attention.
The current British-issued Health Insurance Cards cease to be valid
This change affects primarily British tourists and individuals insured in the United Kingdom. After 1 January, it will no longer be possible to treat patients who carry an old European Health Insurance Card – these cards, bearing the EU logo, are being replaced by new ones which are furnished with a hologram of the Union Jack and the designation "CRA". These new cards began to be issued in November 2020, and can be applied for online following this link to the NHSBSA: https://services.nhsbsa.nhs.uk/cra/start. Insured individuals may also apply online for a paper-form substitute certificate of their claim to healthcare.

Do you intend to go visit Britain or Northern Ireland around the end of the year? Or are you planning to travel to the UK during January? In both cases, you will have to allow for the possibility that the questions surrounding health insurance cards still won’t be entirely resolved. We will continue to post up-to-date information on the website of the ZP MV ČR as it becomes available; at the same time, we recommend that you follow the website of the Health Insurance Bureau (https://www.kancelarzp.cz).
If you plan a trip to the United Kingdom during this period, please do not hesitate to consult us as your health insurer to get the latest information on the possibility to use your European Health Insurance Card, or to obtain a special claim certificate for the UK. We also advise our clients to take out travel insurance before their departure, for the event that they may have to pay healthcare expenses out of pocket which ZP MV ČR finds itself unable to refund.
Are you a British citizen who is employed or is doing business in the Czech Republic?
Under the exit agreement, it is of key importance whether your employment existed before 1 January 2021. Nothing changes for British citizens who have been employed in the Czech Republic since at least 2020, whether during their continued employment or after its termination. It follows from the exit agreement that you will still be subject to the principles following from EU membership after you cease to be employed in the Czech Republic. For instance, you may be health-insured in the Czech Republic based on your residence in the country, without requiring permanent residence.
If you begin your employment in the Czech Republic only in 2021, the exit agreement does not apply to your situation, which is why you will need permanent residence if you wish to remain enrolled in the Czech health insurance scheme.
Analogous conclusions apply to self-employed individuals – entrepreneurs. If you did business in the Czech Republic already in 2020 or earlier, nothing changes for you, even if you decide to discontinue your business. If you begin your operations in the Czech Republic only after 1 January 2021, however, the exit agreement does not apply to you, and you may be treated as a third-country foreigner from outside the EU.
